I had the same issue, so I followed this thread with interest.  I think the
problem was caused by something from a previous version remaining in
.../site-packages/quixote.   Recursively deleting that subdirectory and
reinstalling Quixote fixed the form_demo page.

So thanks Bud, you helped me get this fixed.

Oops,  I think I found the problem and it's all mine:  I have a
different python in the path as super user and thus probably have a
mismatch of versions used for installation (setup.py) and running
it...
